1. Program Description

Sculpt U Club Physical Therapy offers SMS messages to provide updates, promotions, appointment reminders, and important notifications related to our services. By opting in, you consent to receive recurring text messages from Sculpt U Club Physical Therapy to the mobile number you provided.

2. Opt-In & Opt-Out

Opt-In: You can subscribe to our SMS program by checking our “opt-in” box when filling out the form.
Opt-Out: To stop receiving SMS messages, reply STOP to any message you receive from us. You will receive a confirmation message confirming your opt-out request. After this, you will no longer receive text messages unless you opt in again.
